Mammogram and Colonoscopy Screening Form

CLIENT INFORMATIONS
MAMMOGRAM (FEMALES ONLY)

A Mammogram is an X-ray and captures detailed images of breast tissue. It is a screening test to look for early signs of breast cancer. If you are over 40 years old and it is clinically indicated this will be performed at a nearby separate clinic on the same day/day after your health assessment.

Colonoscopy (Males and Females)

Bowel cancer is one of the most common cancers in the Western world and is largely preventable with regular screening. This involves dietary modification beforehand and taking bowel preparation. It is an invasive test, but it is the gold standard test for bowel cancer. If you are over 40 years old and it is clinically appropriate, you can have a colonoscopy as a screening test on a different day at a nearby separate centre of excellence.
